You can save time and enable us to serve you better by downloading and completing forms before arriving for your appointment. Following are forms which you may download for printing at your home or office if you have Adobe Acrobat Reader, which you can get for free.

Before Your Appointment
We strive to make your appointments as convenient and timely as possible. To help us do this, we ask all patients (or their guardians or family members who may be bringing them to their appointment) to please take the following steps and remember the following things before coming in:
  1. Please feel free to call and confirm your appointment. Also, please be familiar with the location of your doctor's office. Click here for telephone numbers, maps, and directions.
  2. Your appointment may involve the use of drops to dilate your eyes. Once dilated, your eyes will be very sensitive to light for several hours after your appointment. Because of this we recommend that someone drive you home from your visit. If this is not feasible, we offer complimentary disposable sunglasses.
  3. Bring along a list of current medications including dosages and how often you take them. If easier, bring your medications and instructions with you;
  4. If you wear eyeglasses, bring them and your most recent prescription if available;
  5. Please bring your valid insurance card(s);
  6. Because co-payments and other amounts may be due at the time of service, please bring cash, credit card, or personal check. See Payment Policies for more information. We gladly accept any of the following credit cards:

  1. Your completed information forms (see below). If you are unable to complete these before coming to your appointment, please leave sufficient time before your appointment (approximately 20 minutes) to complete these in the office.
  2. It is also helpful to have with you the name and address of your primary care physician. If your insurance coverage requires it, please bring a written referral from your primary care physician.

Duration of Your Appointment
Appointments vary in their length depending upon the nature of the visit. A complete examination may take 60 to 90 minutes while a follow-up appointment may require less than 15 minutes.
